Download, print-out, fill-in, and sign this complete Registration Packet (ten pages). Attach a copy of the front and back of your medical insurance card. The due date for all forms and team dues ($200 payable to "Bowie Women's Lacrosse") is November 1, 2008. Turn in the completed packet and check to Team Sponsor, Coach Rasmus in Room E-218 or to your team mate Jennifer Merritt at practice. All team members must also join US Lacrosse. Establish or renew US Lacrosse Membership - $35 paid online.
